Salivary gland imaging is an essential diagnostic tool for evaluating disorders affecting the salivary glands, which include the parotid, submandibular, and sublingual glands. These glands are vital for the production of saliva, which aids in digestion, maintains oral health, and facilitates speech. Various imaging techniques are employed to assess these glands, each offering unique benefits and diagnostic capabilities.
One of the most commonly used imaging modalities is ultrasound. It is a non-invasive, cost-effective, and readily available technique that provides real-time images. Ultrasound is particularly useful for identifying salivary gland stones (sialolithiasis), cysts, abscesses, and tumours. Its ability to distinguish between solid and cystic masses is invaluable in the initial assessment of salivary gland pathologies. Additionally, Doppler ultrasound can evaluate blood flow, which is crucial in differentiating between benign and malignant tumours.
Magnetic Resonance Imaging (MRI) offers excellent soft-tissue contrast and is another vital tool in salivary gland imaging. MRI is superior in detailing the extent and nature of neoplastic lesions, such as benign pleomorphic adenomas and malignant carcinomas. It is also beneficial in identifying inflammatory conditions like Sjögren’s syndrome, which causes chronic inflammation of the salivary glands. MRI sialography, a specialised technique, can visualise the ductal system without the need for contrast agents, making it a safer option for patients with allergies.
Computed Tomography (CT) scans provide high-resolution images and are particularly useful in assessing the extent of salivary gland tumours and their relation to surrounding structures. CT sialography, which involves the injection of contrast material into the ductal system, can highlight obstructive pathologies such as stones or strictures. This technique is valuable in planning surgical interventions.
Nuclear medicine imaging, including scintigraphy and positron emission tomography (PET), is employed for functional assessment of the salivary glands. Scintigraphy can detect reduced gland function in conditions like Sjögren’s syndrome and help differentiate between obstructive and non-obstructive causes of gland enlargement. PET scans, often combined with CT (PET/CT), are instrumental in staging and monitoring head and neck cancers involving the salivary glands.
